Discharge Procedures—Tainan Sin-Lau Hospital

1.Verification from the doctor
May Be Discharged’ order slip from the doctor wait to be called by the Ward Clerk bring discharge slip to 1st F Registration windows 7 or 8 for processing and payment queue up at the Pharmacy for your medications.

2.If patient and/or relative requests for a discharge
●Regular working day
Notify your nurse-in-charge fill up a ‘Against Advise Discharge’ form proceed to 1st F
Registration windows 7 or 8 for processing and payment.
●Discharges on “After office” hours, Sundays and Holidays
Bring the Discharge slip to the 1st F Registration to process ‘partial payment’ discharge procedures.
Return on a regular working day for the completion of the discharge.

3.If you need a Medical Certificate and/or X-ray copies, please notify the doctor or nurse-in-charge in
advance.Bring the documents together with the discharge slip to the 1st F Registrations window 7
or 8 for processing, stamping, and payment.
